Fee Schedule

Many of our programs are covered by most major health insurance plans, however only a portion of your costs may be covered by insurance. The amount covered varies from plan to plan.

Some programs, such as anger management, alcohol/drug information school and problem gambling, in general, are not covered by health insurance plans. Additionally, we do not bill insurance for assessments. However you will be provided a statement that you may submit to your insurance company for reimbursement under the terms of your insurance policy.

We do our best to estimate the cost of treatment at time of admission. However changes during the course of your treatment, such as relapse, or changes in treatment modality will change the costs of your treatment.

Appointments cancelled with less than 24 hours notice result in a $25.00 missed appointment fee.

Insurance
Clients with insurance are required to pay co-payments and deductibles in full at the time services are provided.

Your specific policy is an agreement between you and your insurance company. We do not investigate why a carrier has not paid or why payment was less than anticipated. If an insurance carrier has not paid within 60 days from our submission of your claim you will be billed for those services. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated.

Our Business Manager can help determine insurance eligibility or where insurance is not available, help make financial arrangements.

Sliding Fee Schedule
All our services, except Alcohol/Drug Information School and Assessments, are available on Sliding Fee. Sliding fee is determined by your income. In other words, if you have very low income you will be asked to pay less for a service than someone who has more income. Unfortunately, even the lowest fee may still be high for a person with limited income. There is no guarantee that a service fee based on a sliding scale will actually be affordable to everyone.

To find out if you qualify for sliding fee services, you will need to provide proof of address, photo identification and proof of income.
